What Would Make A Lower Arm Washer Do This...

Took the lower arms off yesterday, newly fitted 309 ones. I had stuck washers on everything and the forward side of the rear fixing one came off looking like this, any ideas why that might be? It had only done about 300 miles.

Guess it might have been overtightened but I've never seen this before, interested to hear your thoughts, to me it seems as though something was moving...

Looks overtightened, also could be elongated holes in the subframe allowing the bolt to move up and down??

I'll have a measure up and see. Was fine before, I'll be gutted if the subframe holes are no longer round!

I did get the washer from a nut and bolt store, figured they are all the same
