Juddering

Hi guys,

 

Over the past 2 months while accelerating gradually from 2nd gear up to 5th everytime i hit around 2000revs it starts to judder as if there is a loose connection somewhere. If i accelerate quickly it doesnt happen, iv changed the plugs, and still made no difference??

 

Any ideas?? HT leads?? Weak mixture??
